Removed replace, fallback & decorate methods from Setup

Judgement call on convenience methods. These methods might be
useful for development environment, but they also open many
new possibilities to introduce configuration problems for
unfinished production code.

Configuration constraints are more strict, but simple:
App won't work if it's not configured correctly.
