Fix -o to not outpt to user store

The -o option was inadvertently writing {server}.pem files to the users
config folder. This would override the users current PEM which was not
intended. -o will not output only to the desired output location.

A few checks were also added:
* The output location must either be a relative path or start with /
* the output location must end in a pfx
