* basic CollectionIndexer

* basic indexing for models and metastreams, with specs

* fix superclass for WorkflowIndexer and AdministrativeIndexer

* use more AR-ish patterns for institution and exemplary_file_set methods

* explicitly create exemplary_object when creating :curator_mappings_exemplary_image factory

helps avoid uniqueness errors on Curator::Mappings::ExemplaryImage

* use sequence to ensure ark_id uniqueness for :curator_digital_object factory
