Make a Fathom bundle suitable for use internal to Firefox. Close #165. Ref https://github.com/erikrose/gecko-dev/pull/6.

* Remove references to `document`, which make Firefox throw errors.
* Provides a way to access submodules like `utils` and `clusters`.

As bonuses:
* Drops from 237K to 105K unminified by removing wu, the sole front-end runtime dependency.
* Update rollup, the coverage tooling, Babel, the linter, and the testrunner.
