Increase the max_length of various fields

@edent reported this issue - the example was Professor Andrew Knight
whose post-nominal letters are:

  BSc (Vet Biol), BVMS, CertAW, DipECAWBM (AWSEL), PhD, MRCVS, SFHEA

As well as increasing the max length of the honorifics field, increase
the length of full name field as well, just in case.
