Add username to the SSH command when forwarding ports (#30)

* Add username to the SSH command when forwarding ports.

This previously worked only because our local username was the same
as our username on the tunnel host (EB).

Also, fix a bug in forward_test.go where the test could get stuck
if environment variables were already set to non-empty values.
