A second run of refactoring for 0.12 release

- Add factory functions and request with failsafe context. Should resolve #3
- Restructure requirements to test, dev, main
- Move extension to main.py
- Restructure tests and add constants
- Add Makefile and commands for tests, run, install
- Add regex support to bypass and bypass_caching
- Genreal code-refactoring and missing comments
- Update README with latest changes
- Use endpoints instead of url.rule for caching
- Add missing unit tests to increase coverage
