feat: split state into "value" and "status"

BREAKING CHANGE:
* The "state" property is no longer supported. Please use the "value" and "status" property instead.
* The "onChange" callback now emits just the form value instead of the form state. If you want to subscribe to form status updates, use "onStatusChange".
