Grid refinement and simulation fixes (#242)

* Use file context manager for grid refinement output file.

* Fix connection issues for time and design parameter related targets in the ODE during grid refinement eval_ode.

* In the SolveIVP transcription used by simulate, each segment now has its own instance of ode_integration_interface rather than sharing a common copy.  This was
causing errors when attempting to interpolate controls in phases with varying polynomial orders.

* GridData transcription_order now casts lists and other sequences to np.arrays.

* Added a test to verify that simulation of phases with varying polynomial orders works.

* ph adaptive grid refinement now correctly populates inputs when evaluating the ODE for error.
