* Add support for optional index prefixes

This is useful in multi-tenant environments where a single Elasticsearch cluster is shared

* Add unit tests for IndexNamePrefixer

* Fix double prefixing in UpdateIndexAliasStage

Added a test for this too

* Add README instructions

* Change the elastic:migrations:create output message to be slightly more informative
