Initialise pcp::instance_domains private members

As identified by Coverity Scan (issue 89191), the compiler-provided
default constructor was not initialising the two provde ID members.
This doesn't cause issues in the standard use pattern, since sensible
calling code always sets the first ID, while pcp::pmda always sets the
second ID. However, intialising both IDs to PM_INDOM_NULL is good
defensive programming.

Also, instance_domain::pm_instance_domain test, which was checking the
min edge case twice, instead of testing min and max.
