CA: Add SubjectKeyId to certificates. (#269)

According to https://tools.ietf.org/html/rfc5280.html#section-4.2.1.2 all
certificates including the Basic Constraints extension should have
a SubjectKeyId. This PR makes sure the certificates generated by Pebble have
one.
