Commit Message

Move confirm page to instance subdomain and change url

This is the page that the user gets to after clicking the confirmation
link in the confirmation email. This now lives on the subdomain so it's
associated with the correct writeit instance.

I've also changed the url to match the new url scheme that we've
introduced.
