Use bool in HTML2Text.o() rather than int 0/1

The value is being used as a boolean for branching, so use a boolean
rather than an int. Easier to understand for the reader.

Additionally, always use a keyword argument rather than a bare 0/1 or
False/True to provide context to the reader.
