Make use of setuptools_scm in versioning

Keep the date-based YY.MM main version scheme but represent the
distance from the last release with an exact number (counting
additional commits) instead of a vague "+x". For the sake of
identification, non-released versions also encode the commit hash
in the local part of the version.
