Avoid putting error patterns into lexer

- Rather than defining the regex that will be used to determine what
  kind of error for a given input fragment directly into a real lexer
  rule that happen to fail (instead of producing a real token), have
  the t_error token error handler iterate though a list of callables
  that may handle the specific cases.
