Clean up how we handle `rec_types` repitition internally, close #141

rec_type levels and groups are now consistent and defined in internal data,
addressing #141. As part of this we have new internal functions to take
advantage of this for IO (rec_type2abrv() and abrv2rec_type()). This is also 
the basis for several internal changes to util.R functions. The new internal
data include vectors: rec_type_all, rec_type_abrv, rec_type_recorder, 
rec_type_injury, rec_type_scar, and rec_type_ends. Advanced end users
can also access these with `burnr:::rec_type_all` notation, to get the
rec_type_all vector, in this case.

Also, have minor correction to bad as.fhx() documentation.
