Mutable tensor for local session and web session (#536)

* Mutable tensor for LocalSession.

* Mutable tensor for web session, and a bit refactor.

* Skip error message checking for web session.

* Return the mars web api exception in a proper manner.

* Revise the web endpoint for mutable tensor.

* Test _dump_exception of web API.

* Revise the doc string, and add mt.mutable_tensor constructor.

* Add `mars.tensor.core.mutable_tensor` constructor.

* Remove the `Index` definition inside the `MutableTensor`.

* Implement `fill_value` parameter for mutable_tensor constructor.
