Commit Message

Port MachineController to SCPProtocol

* SCPConnection has finally been removed
* The API remains pretty much unchanged for the block-happy user.
* Everything except the MemoryIO and boot ocmmands now optionally non-blocking
* All internal implementations queue requests in parallel where
  sensible/possible
* Currently only one outstanding command at a time is used since functionality
  for detecting how many required is missing.
* Moved internal implementation a baby-step towards multiple Ethernet
  connections
* Adds simple utility functions for mocking async stuff
