Two factor naming improvements. (#104)

Unify and clarify form, endpoint, model, method naming for two factor feature.

In a couple places - replaced 'flashing' with returning errors in forms.
Flashing is really just for when redirecting.

Main changes:
- User model - prefix two factor attributes with 'tf_' - THIS IS A BREAKING CHANGE.
- 'change method' really meant 'setup' - so change accordingly
- 'change_method_verify_password' was so long it couldn't be properly formatted!
  and it really is just 'verify_password' - changed accordingly.
