Fix quote and bracket processing around span tags

Do not unintentionally eat brackets from around span tags, unless
it is a parse enforcing square bracket, extending the intended link
functionality to span and inline tags, including notextile and
escaping syntaxes.

Previously using the square bracket feature was possible with span tags
too, but it was due to a bug with span quote handling, which resulted
the span tags eating any supported quotes from around the tag. Now it is
an intended feature.

Other quotes and brackets also allow span rendering mid-word, but they
are kept in the resulting output regardless whether used mid-word or
otherwise.

This allows using square bracket wrapping to get the output intended in
the issue #191, while keeping the current inline span tag processing the
same; mid-word syntax is ignored to avoid false syntax matches unless
enforced with square brackets.
