Fixes issues with decoding Multipass payloads

v0.1 didn't correctly handle payloads generated by the Multipass
Ruby gem which used the `:url_safe => true` keyword arg. This is
the default action of the Ruby gem. Error structs were added to
aid with readability and documentation added/updated.
