Rejection can raise and report errors to NewRelic

These behaviors are determined by the current Rails environment.
Also in this commit:
* File processing in transactions has been moved to its own operation
* Rejection has been refactored to accept the actual Error object
* ChangeSetPersister#validate_and_save now does not blanket rescue all
  errors
