Fix SOL regression in PHP tokenizer due to extTagOffsets patch

This fixes a regression in the PHP tokenizer introduced by
I4c5437a01 /
6700f2673, which was partially
but not completely fixed by
Ie8d1218cf /
2c35180e7.

Same root cause: I forgot a string argument to substr in both cases.

No new tests needed because this bug was found in the first place
because of a failing test ("Includes and comments at SOL").
