Test integration with various markup processors

YARD supports several different markup languages, and plenty of markup
processors.  However, their integration was hardly tested.  This is now
being fixed.

Every test suite contains an example document written in respective
markup language.  Then it is tested if specified markup processor
correctly translates that document to HTML.  And finally, it is tested
if YARD's enhancements are applied correctly, most importantly syntax
highlighting.

Some tests are pending, which reveals new bugs.
