Custom webserver headers are not added to OPTIONS requests (#596)

* Fixed bug where user-defined headers were not being added to OPTIONS requests.
* Added associated tests.
* moved from headers being map[string]interface{} to map[string]string.
* removed header typecast.
* added error checking to server instantiation routine (including lambda)
* added empty header runtime warning
