Constrained variables are automatically added to timeseries outputs (#161)

* State scaler, defect_scaler, and defect_ref now default to None, the OpenMDAO default.
If defect_ref/defect_scaler is unspecified, the value will be based on the variable value scaler/ref.

* Boundary and path constraints on ODE outputs are now automatically added to the timeseries outputs.

* Those constraints on states, times, controls, etc are already included in the timeseries output and are unaffected.

* Added check_config to phase/transcription to warn of path/boundary constraints or timeseries outputs on ODE outputs for which no explicit shape is provided.
This is the easiest path for most use-cases when the outputs are scalar.  Connection errors will result when the outputs are shaped, but the user should note the warning.
