fix: Fix validation of required fields in presence of 'readOnly' on nested schemas.

Where a schema had nested values, or referenced other values that were of the form `{ required:
['foo'], properties: { foo: { readOnly: true } } }`, we would require the readOnly value to be
present in a request.  (Similar for writeOnly in a response.)
