* 💥 add opentracing support to Remit

Pass in a tracer as an option when creating a Remit instance and watch it fly!

* ✅(emitter): remove redundant `started` item in event

* ✅(tracing): remove all redundant (old) tracing tests

* 🚩(events): reintroduce `started` in events so as to not make a breaking change

* remove unneccesary imports

* 📖 add opentracing badge to README

* 🚩 use Remit-scoped namespace instead of global bubble

This also allows users to pass in the namespace used for this, letting them sync up the usage across multiple libraries.

* (utils): remove unused bubble util

* 📖 add tracing to hosted docs

* docs: fix bad highlighting syntax
