In phpcs, exclude "missing documentation" rules for all but src/

* Exclude them for tests/ where they are less useful and are likely to
  inherit docs from the parent classes a large proportion of the time
  anyway.
* In addition, exclude Grammar.php from phpcs checking since that is
  wikipeg-generated.
