Run flake8, isort, and black on all source files

Including test files and setup.py. All Python code can be formatted
consistently and checked for errors. This change also:

- Unpins flake8 version in tox. Now run against the latest version.
- Simplifies flake8 configuration:
  - Ignore fewer errors and warnings
  - Use the default exclude list
  - Remove `changedir = {toxinidir}` from tox.ini as it is the default
