README: update application description and acknowledgment (#53)

As our application has morphed from an Address Book application to a 
degree planner application, the description of the application on 
`README.adoc` does not match what our newly morphed application does.

Besides that, we also need to credit the original source of Address Book
(SE-EDU initiative).

Let’s update the description to include the features of our application 
and also update the source code acknowledgement to SE-EDU accordingly.
