improve axis3 handling

Remove the need to patch the WCS header to make it 3D, even if one is
only interested in a 2D map. Can now specify a keyword-arg ('naxis3')
for that. Furthermore, the grid-method automatically adds a 2nd dimension
to the signal and weight arrays, if necessary.
