Docs generation tweaks

- Removed static table of commands and replaced it with a simple CSV generated
  from a few simple line of Python code (much more maintainable.)
- Moved all generated files into docs/source/dyn to make a clear distinction
  between what docs should be edited directly vs which are auto-generated.
- Add a simple ReST filter into MyDescriptionHelpFormatter (argparse text
  formatter that strips out a few very simple styles of markup and leaves us
  with appropriate plain text version for the CLI docs.  Nice!
