Completed docs refactor (ready for review)

- Complete migration from Markdown docs to reStructuredText.
- Updated CLI update tool to generate RST.
- Created new pages and refactored content to cover highlevel concepts and
  random topics.  I also wrote a bunch of new stuff and thew out some trash.
- Completed first pass of new docs for all commands.
