Basic moderation backend (#328)

* permissions as column in user table - simple

* superuser or moderators can edit/delete comments, questions and reviews

* updated tests to use common auth token generation code, secrets now env variables

* local db password different from azure db password

* admin/superuser added, with delete comment test

* removed nondeterminism from review GET test

* reduced mocha test timeout to 10 seconds
