Introduce ContextObserver API

Allow `ContextObserver` interface to be notified with context updates
for particular (or all) ContextManagers.

This fixes #91

Responsibility for observable contexts lies with the context implementor
but the `AbstractThreadLocalContext` has already been extended with this
functionality.
