Cleanup and Improve README

A few things were ugly or outdated in the README; some still are, probably, but here are the ones I've addressed:
- Badges updated for consistency with Parser
- Added mention of Parser for those who might be interested
- Simplified instructions for adding Composer requirement
- Added note to Laravel users reminding them how facades differ from the examples
- Fixed header levels to reflect actual nesting depth
- Renamed "Registering the CustomFormatter" to reflect why rather than what
- Duplicated closing sections from Parser for reference
