Remove .bind() usages from lib/wt2html except TokenTransformManager

* My first attempt at removing bind usages in TTM caused mysterious
  failures, so this patch is leaving them out. The async bits of TTM
  will anyway get dropped on the floor during the port so that is
  less critical.
