Support more natural spec definitions with x-subspec

- x-subspec uses a full swagger spec including the top-level info, which makes
  the definition and merging of top-level specs more natural.

- Allow x-subspec and x-subspecs at the spec top-level to avoid introducing an
  extra level in the hierarchy.

- Use this to build separate merged specs for /{domain}/v1 and /{domain}/sys,
  and remove hack that previously filtered out /sys.

Next steps:

- Expose the spec at ?spec
- Hook up swagger-ui at ?doc, using ?spec
