Use nyc for coverage, update mochify and browserify (#1950)

* Upgrade Browserify and Mochify and introduce nyc

- Mochify v6, including Mocha 5 and Browserify 16
- Browserify 16 to match Mochify
- Replaced mochify-istanbul with nyc, @babel/core, babelify and
  babel-plugin-istanbul
- Configured nyc and reworked test-coverage script

* Upgrade Puppeteer to match Mochify

With different versions, the post install script is executed twice
causing two Chromium downloads.

* Upgrade Mocha to v5 and fixup Mochify

Branch coverage included in aggregate %.

1941 of 2008 relevant lines covered (96.66%)
