Fix handling of setting variable bounds.  (#81)

* Fix handling of setting variable bounds. We need to loop through and
collect upper and lower terms, otherwise we can end up trying to set
variable bounds with lower > upper.

* Add a pre-solve check for infeasible bounds on variables and rows.

* Remove bounds check and return InvalidModel instead of trying to intuit that the problem is infeasible.

* Remove aggregation of variable bounds
