Make identity environment variables more consistent.

The new `$SQITCH_USERNAME` environment variable complements the existing
`$SQITCH_PASSWORD` environment variable, superseding any value from a URI.

Rename `$SQITCH_USER_NAME` and `$SQITCH_USER_EMAIL` to `$SQITCH_FULLNAME` and
`$SQITCH_EMAIL`, respectively. These names don't aline quite as well with the
`user.name` and `user.email` configuration settings, but better complement the
other environment variables. `$SQITCH_USERNAME` and `SQITCH_USER_NAME` would
have been too confusing.

Rename `$SQITCH_ORIG_NAME` and `$SQITCH_ORIG_EMAIL` to `$SQITCH_ORIG_FULLNAME`
and `$SQITCH_ORIG_EMAIL`, respectively. These variables are designed to directly
correspond to  `$SQITCH_FULLNAME` and `$SQITCH_EMAIL`.

Add `$SQITCH_ORIG_SYSUSER` as the value to be passed from an originaing system
to an execution system.
