feat(action-items): add more error handling (#839)

* refactor(action-items): consolidate error handling

There's an action-items array and an error array. The state goes into the arrays. The view renders the arrays.

* feat(action-items): consider bad item config a user-facing error

Tell the user what they're not seeing because of misconfiguration.

* docs(action-items): change partially-broken example to be more relatable

* feat(action-items): treat no data cases as user-facing errors

Tell the user what we're not able to show them.
