Use nout=2 in ndmeasure's label (#67)

* Create delayed label function object

Split out the creation of the delayed `scipy.ndimage.label` function
from its use in `dask_image.ndmeasure.label`. Makes things a little bit
more readable.

* Use `nout=2` with delayed label function

Instead of selecting out each value from the `delayed`, simply specify
that the `delayed` wrapping of `scipy.ndimage.label` returns 2
arguments. This simplifies the syntax a bit. Also has the potential to
benefit from any optimizations Dask might make to the `nout > 1` use
case.
