Fix Heroku setup script issues.

The figaro command needs to include the app name in the scenario where
you have multiple Heroku apps in your account. Figaro also expects all
ambiguous env vars to be surrounded in quotes, i.e. those that could be
mistaken for integers or boolean values.

The DB setup script only needs to run the migrations, so I added that
command in `setup_heroku` and removed `setup_prod_db`.
