Infer annotation types (#323)

* Adds inferred types to annotation columns in BQ.

Uses a first pass over the variants to create info headers in the 
representative header file encoding the annotation types.
These headers are later read while creating the BQ schema.

Fixes: #239
Tested: unit tests

* Updates based on PR review.

Moved infer annotation type logic into InferHeaders.

* Changes from second round of review.

* Added `annotation_fields_to_infer` for `InferHeaders`.
* Stopped inferring `ALT`

* Changes from third review, and actual type casting.

* Several more nits from review.

* Four more small updates.
