Make extracting let bindings much more robust

Move all the necessary logic into emacs-refactor, and add tests. Fix
the bugs found.

Remove dependency on redshank, which did not handle as many situations
and was unmaintained. Remove backward-down-list compatibility function
which was just for redshank.
