Issue #2984453: prepare for Drush 9 / Drupal console. (#48)

- fix settings extraction.
- add @covers/@coversDefaultClass on all test classes.
- fix find() serialization failing on MongoId objects.
- convert collection count from a Logger protected method to MongoDb::collectionCount()
- support PHP 7.2.
- refactor more drush-8 specific code.
- use standard location and namespace for kernel tests.
- add KV commands test.
- exclude tests from coverage. Fix watchdog test location/namespace.
- test most-ikv import of persistent KV data.
- one pass of coding standards/PHPMD.
