Refactor the versions to be integers

All versions are now represented as integer milliseconds
since the UNIX epoch. The only place they are strings is
when they are used as the keys in the diffs dict in mongo.

Other changes include:
    - slight tidy up of a couple of todos
    - fixing of index mapping
    - addition of a to_timestamp function for common timestamp
      creation
